Please read our following policies and procedures. Payment Policy: Payments are required to be paid in full prior to your appointment date. Appointments that are booked over the phone will receive a invoice from Dose of Wellness Massage & Facial square account. The invoice would have to be paid in full the same day that it is sent out. Successful payments confirm & holds your appointment slot. Non-payment will result in your appointment being canceled. Arrival Policy: Please arrive 15 minutes prior to your appointment time. This gives you time to park and fill out your client health history form. All services have a specific starting time. Early arrival will allow you to experience your entire session. Late arrival will result in a shortened session, however you will be charged for the entire session. Cancellation Policy: If you have to cancel your appointment please call 24 hours in advance. Anything pass that time will result in a $25 rebooking fee. We do not issue refunds, however you do have the option to reschedule your appointment. A no call, no show will result in your appointment being forfeited and possibly no future bookings. Professional Boundaries: Upon arrival, you will receive a health form to fill out. This form also complies your consent. At the beginning of each session the practitioner will discuss your health form with you. Practitioners are not allowed to do anything outside of their scope of practice as well diagnose or prescribe any medications. Practitioners are allowed to refer you to your doctor for diagnostics or medications. Practitioners are also able to end a session if they feel uncomfortable at anytime. Client Needs & Boundaries: Clients are able to leave on as much clothing as they like for comfort. Clients are also able to end a session at anytime or refuse any technique. Sexual Behavior: Sexual behavior will NOT be tolerated at Dose Of Wellness from either party, Client or Practitioner. Sexual behavior from a practitioner will result in immediate termination. Sexual behavior from a client will result in them no longer attending Dose Of Wellness. Age Policy: if you are under the age of 18 and would like to book a session, a consent form from your parent is required. Existing And New Medical Conditions: Clients are responsible for informing their practitioner of any new medical conditions or any medical treatment that is being taken.
